Semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) are the two most consequential weight-loss medications ever developed. With 2026 trial data now mature, here is what head-to-head evidence actually shows.

For peptide context, see our peptide dosage chart and our complete peptide guide for beginners.

How They Differ Mechanistically

Semaglutide is a GLP-1 receptor agonist. Tirzepatide is a dual GIP/GLP-1 receptor agonist — the added GIP activity enhances insulin sensitivity and appetite suppression beyond what GLP-1 alone achieves.

Head-to-Head: SURMOUNT-5 (2026)

Eli Lilly’s SURMOUNT-5, the first direct head-to-head trial (n=751, 72 weeks): tirzepatide 10/12.5 mg averaged 20.2% weight loss vs semaglutide 2.4 mg at 13.7%. Waist circumference: tirzepatide -18.4 cm vs semaglutide -13.0 cm.

Earlier Trial Data

STEP-1 (semaglutide, NEJM 2021): 14.9% mean weight loss at 68 weeks. SURMOUNT-1 (tirzepatide, NEJM 2022): 15 mg dose averaged 20.9% at 72 weeks.

The 2026 Landscape

As of 2026, Eli Lilly’s oral orforglipron and Novo Nordisk’s oral amycretin are advancing through Phase 3, and retatrutide (triple agonist) has shown even larger reductions in early data.

Side Effects Compared

Both: nausea, vomiting, diarrhea, constipation, decreased appetite (usually mild-to-moderate, fading over weeks). Semaglutide carries an FDA boxed warning for thyroid C-cell tumor risk (rodent data; contraindicated with personal/family history of medullary thyroid carcinoma or MEN 2); both carry pancreatitis, gallbladder disease, and gastroparesis warnings.

Cost and Access (2026)

List prices: Wegovy ~$1,349/month, Zepbound ~$1,059/month; most insured patients pay far less. Medicare still largely excludes weight-loss coverage; Medicaid varies by state.

Who Wins?

On efficacy, tirzepatide wins the head-to-head. On tolerability and individual response, it varies — some patients do better on semaglutide. Both require long-term use; stopping typically restores most lost weight within a year.

Frequently Asked Questions

Which causes more weight loss? In SURMOUNT-5, tirzepatide averaged 20.2% vs 13.7% for semaglutide.

Are oral versions available? Oral orforglipron and amycretin are in Phase 3 as of 2026; not yet approved.

Do I have to take them forever? Evidence suggests weight regain after stopping; treat as long-term therapy with your physician.
